Policies
We maintain the following policies, described below for the convenience of our clients. NEW ACCOUNTS
For your protection and for the best interest of our business relationships, we request that all new clients complete a credit application. Until a credit application is approved, we will limit new accounts to a credit line maximum of $500.00. Accounts over the maximum will be C.O.D. to the extent they exceed this limit.
BILLING POLICY
Customers will receive monthly statements reflecting charges for the current billing cycle. An account is considered past due when we do not receive full payment within 10 days. Payment in full is due upon receipt of statement. We charge all past due accounts an additional 1½% service charge on the unpaid balance each month (18% APR).
POSTAGE
Upon request, we will provide pre-addressed and prepaid labels for shipping. There will be a slight charge for postage and handling. Call our office at (888) 238-2685 to determine the best delivery method.
IMPLANT SERVICES
We bill all fixed implant work at base labor plus alloy and components. Case design and manufacturer of components will dictate the total cost of implant cases. We will provide a free written cost estimate upon request.
INFECTIOUS DISEASE CONTROL
We process every case in our laboratory. Every case processed by our laboratory is processed through an Infectious Disease Control System. We disinfect each case entering and leaving the laboratory in accordance with all state and national guidelines.
GUARANTEE POLICY
We guarantee our products for one year from date of invoice. We do charge for remakes under the following circumstances: § We question a die, model, or impression and the doctor okays completion. § We request a try-in and the doctor says to complete case. § We are to remake to a new bite or if we must re-prepare a preparation. § We receive no study model for anterior cases and/or we receive no specific directions. § A shade is different from the original request. We do not guarantee repairs on old appliances. |
